Pre-registration will enable our staff to complete your registration, obtain needed medical tests, complete a medical history, and prepare you for your surgery.

Our staff will be happy to assist you in completing the simple registration process. If circumstances prevent you from coming to our facility to pre-register, you may call and pre-register by phone at (406) 556-9000.

If custody or guardianship of a patient has been awarded by a court of law, court papers must be presented prior to surgery.
